Complaints Form

Please review our step by step guide to complaints, and the Guild's policy on complaints, before completing this form - as this will likely save you time. You can receive informal advice on any complaints from a Guild staff member or officer, however all formal complaints should be made via this form. We cannot accept any anonymous complaints.

We will treat all information collected during any complaints process as confidential. However, if there is evidence of potential criminal activity or harm to an individual or a group of people, UCB Guild reserves the right to break confidentiality. If there is a need to break confidentiality in any situation, this will be approved by a senior leader within UCB Guild, such as the President or Guild Director. Information will be kept for monitoring purposes and will be stored securely, in line with UCB Guild's Data Protection Policy, and destroyed after seven years.

You may withdraw a complaint at any stage of the process. Your withdrawal must be submitted in writing to the lead investigating officer. Should this be the case, the Guild will assess the information to identify risks or legal responsibilities. If there is an indentified risk, we may continue to investigate and will retain information as outlined above.

Please submit the form below in full. You will be contacted within five working days by the lead investigative officer, who will be your point of contact throughout your complaint investigation.
